I understand that this is a smoothing and strengthening treatment, not a straightening treatment.
My hair will not be straight unless I use a flat iron after washing. The sleek, glassy hair shown in SMOOTHED’s ads and social media posts has been styled with heat. If I air-dry my hair, it will retain its natural texture, but with reduced frizz, improved shine, and easier manageability.
This treatment does not permanently alter my curl pattern.
I have fully disclosed all prior treatments on my hair, including keratin, Brazilian blowouts, relaxers, Japanese straightening, rebonding, or other chemical services, whether performed in the U.S. or internationally.
I understand that these previous services may affect the outcome of today’s treatment, and SMOOTHED cannot guarantee results if prior chemical history is inaccurate or incomplete.
I understand that keratin treatments may temporarily shift my hair color, regardless of whether my hair is light or dark.
This treatment may:
These shifts are normal and expected with color-treated hair. I’ve been advised to keep a color-safe toning mask or gloss on hand for aftercare, and I understand this is not considered a service error.
I’ve also been informed to schedule color services two weeks before or one week after my keratin appointment for best results.
I understand that keratin treatments work best with consistency. While most clients see a dramatic difference after one treatment, others—especially those with coarse, porous, or chemically damaged hair—may require multiple treatments to achieve their desired results.
SMOOTHED’s keratin treatments are designed to last up to three months when proper aftercare is followed, including avoiding sulfates, saltwater, and excessive sweating during the initial setting period.
Keratin is a cumulative treatment that builds on itself over time, and results may improve with consistent reapplication every 3 months.

I understand that while SMOOTHED uses non-toxic, formaldehyde-free products, reactions or sensitivities may still occur in rare cases. I have disclosed any known allergies or sensitivities to my stylist. I release SMOOTHED and its staff from liability in the event of an unexpected reaction.
I understand that arriving more than 15 minutes late may result in a shortened service or rescheduling at the stylist’s discretion. I agree to give at least 48 hours’ notice to cancel or reschedule. Missed appointments or late cancellations may result in a $50 fee or forfeiting my appointment deposit.
I give permission for SMOOTHED to take before-and-after photos of my hair for internal documentation. I also give permission for these photos to be used on SMOOTHED’s website, social media, or marketing platforms, unless I specify otherwise in writing.
I agree to follow all aftercare instructions provided by my stylist. I understand that exposure to salt water, chlorine, excessive sweating, sulfate shampoos, or improper heat tools may impact my results. I also acknowledge that this is a professional-grade treatment and that my participation in care and maintenance matters.
Clients who have received a keratin, smoothing, or straightening treatment outside of the United States within the last five years must disclose this prior to receiving services at SMOOTHED. Due to inconsistent international product labeling and the use of chemicals not approved under U.S. safety laws, SMOOTHED reserves the right to decline service unless the client signs a waiver acknowledging the risks and differences in expected results.
By booking a keratin treatment with SMOOTHED, I acknowledge that I have read, understood, and agreed to all terms outlined in this waiver, including the refund policy, cancellation policy, and treatment expectations.
I understand that in rare cases, individuals may have a significantly above-average amount of hair (i.e., the equivalent of two to three heads of hair on one scalp). In these cases, the time, labor, and product required may exceed what is covered in the base price of service.

At SMOOTHED, we always strive to provide every client with the highest quality treatment possible while prioritizing the long-term integrity and health of the hair. However, clients with previously chemically treated, over-processed, bleached, relaxed, Japanese straightened, or otherwise compromised hair should understand that their results may differ significantly from the results seen on our social media platforms.
When working on damaged or chemically compromised hair, we are often required to use more customized, conservative, and integrity-focused processes in order to safely perform the service without causing additional damage. This may include lower heat settings, modified processing techniques, reduced smoothing intensity, or other adjustments based on the condition of the hair.
Because of this, clients with compromised hair may experience results that are less smooth, less dramatic, or slower to improve than healthier hair types. Hair that has been heavily bleached or over-processed may still feel dry, fragile, or damaged after an initial treatment, as keratin treatments are not an instant reversal of prior chemical or heat damage.
Keratin treatments are intended to help reduce the ongoing cycle of excessive heat styling, frizz, and further damage by creating more manageable hair over time. In many cases, improvement is gradual and cumulative rather than immediate. Multiple treatments, healthier styling habits, reduced heat exposure, and time may be necessary to see significant improvements in the overall health, softness, and manageability of compromised hair.
While SMOOTHED will always make every reasonable effort to improve the appearance and manageability of the hair while preserving its integrity, specific results cannot be guaranteed when working on previously damaged or chemically altered hair. Individual results will vary depending on the condition, history, and overall health of the hair.

Keratin treatments are cosmetic chemical services. While SMOOTHED exclusively uses formaldehyde-free and California-compliant products, all cosmetic and chemical services carry the potential risk of irritation, sensitivity, or allergic reaction. Reactions may occur even if you have previously received keratin treatments or similar services without issue.
Certain factors may increase the likelihood of sensitivity or adverse reactions, including allergies, skin or scalp conditions, asthma or respiratory conditions, medications, pregnancy or hormonal changes, previous chemical services, undisclosed sensitivities, or prior keratin or smoothing treatments performed outside of the United States.
Patch testing is available upon request prior to service; however, patch tests are not required before receiving treatment at SMOOTHED. By choosing to proceed with services, clients acknowledge and accept the potential risk of sensitivity, irritation, or allergic reaction that can occur with cosmetic chemical services.
Clients are responsible for disclosing all known allergies, sensitivities, medical conditions, medications, previous reactions, and prior chemical treatment history before receiving services. Failure to disclose relevant information may increase the risk of adverse reactions or unsatisfactory results.
If at any point during the service a client experiences discomfort or an unusual reaction, they must immediately notify their stylist so the service may be adjusted or discontinued for safety reasons. Clients may also experience reactions after the treatment is rinsed at home. In very rare cases, a sulfuric odor may occur when hair is wet. While rare, this usually only lasts a few washes. Other clients experience a feeling of hair loss, its not hair loss. Your hair simply takes up less space after a treatment, so ponytails will fell smaller. Some clients experience a feeling of oily hair. Your hair cuticle is sealed down from the treatment and therefore accepts less oil.
SMOOTHED reserves the right to refuse or discontinue services when deemed necessary for the safety, health, or integrity of the client’s hair or scalp.
By receiving services at SMOOTHED, clients acknowledge that they understand and voluntarily accept the potential risks associated with cosmetic chemical services and choose to proceed at their own discretion.
I understand that arriving more than 15 minutes late may result in a shortened service or rescheduling at the stylist’s discretion. I agree to give at least 48 hours’ notice to cancel or reschedule.
As SMOOTHED continues to grow and our waitlist expands, appointment availability has become extremely limited. Because of this, we are implementing stricter policies surrounding late cancellations and no-shows in order to respect both our stylists’ time and the many clients actively waiting for appointments.
We currently do not require deposits for new client appointments because we believe in making the booking process as accessible and stress-free as possible. However, by reserving an appointment, clients are committing to a lengthy service appointment that could otherwise be offered to another guest.
If a client cancels within 48 hours of their appointment, the full cost of the reserved treatment must be paid before another appointment may be booked by said client, at SMOOTHED.
If a client no-shows or cancels within 24 hours of their appointment, they may be permanently restricted from booking any future appointments with SMOOTHED, altogether.
We understand that emergencies and unexpected situations can happen, and we always encourage communication whenever possible. These policies are in place solely because appointment demand is exceptionally high and waitlisted clients are actively hoping for openings.

Smoothed's signature "Curl-Enhancing" keratin treatments are designed to reduce frizz, soften texture, improve manageability, and decrease volume while maintaining as much of the natural curl pattern as possible. However, clients should understand that all keratin treatments alter the hair to some degree and may result in partial loosening, elongation, or reduction of the natural curl pattern.
Some curl loss should be expected as part of the smoothing and de-volumizing process, particularly in areas of the hair that are already naturally looser, more fragile, heat-damaged, tension-damaged, chemically compromised, or less curly to begin with. Certain hair types, including finer or more delicate textures, may be at a higher risk of experiencing additional curl loosening or reduced curl retention over time.
While SMOOTHED will always make every reasonable effort to preserve the integrity and natural texture of the hair based on the client’s goals, results cannot be guaranteed, and individual outcomes will vary depending on hair history, texture, density, heat exposure, at-home maintenance, styling habits, environmental factors, and overall hair health.
Clients should also understand that softened or smoothed hair may become more susceptible to future heat or tension-related damage if proper care is not maintained between appointments. Excessive heat styling, improper styling techniques, over-manipulation, tight hairstyles, poor at-home care, or failure to follow recommended maintenance instructions may contribute to additional texture changes or curl loss over time.
By proceeding with a curl-enhancing keratin treatment, clients acknowledge and accept that some degree of curl alteration, loosening, elongation, or texture change is an inherent and expected part of the service and that SMOOTHED cannot guarantee full preservation of the original curl pattern.
